Guidelines for completing the form
- Complete the form below and submit on our website, or fill-in a form in person at one of our facilities.
- Comments made in these forms will be recorded, documented and are required to be actioned in accordance with our feedback/complaints procedures.
- If you submit a compliment, a copy is circulated for the attention of relevant staff.
- If you submit an opportunity to improve, we will raise the matter in monthly management meetings in which it will be formally considered.
- If you submit a complaint and you have provided your contact details, our management staff will contact you directly to address the matter.
- You may submit the form anonymously if you would like, however it can be difficult to follow-up the matter if we require more information.
- Please specify if you would like us to contact you personally in response to comments submitted.
- You may arrange a meeting with one of our facility managers or phone to discuss the matter over the phone, if you do not wish to fill in the form, but would like to provide feedback.
- If you are not satisfied with your response, you may consult the directors of the business, or failing any satisfaction through our facility’s complaints procedure, you may contact the Aged Care Complaints Commissioner- 1800 555 677- operated by the Australian Government.
